The FDF must be sworn under penalty of perjury. Required disclosures include all income sources — wages, business income, rental income, investment income, and any other regular or irregular receipts. All monthly expenses must be documented. All assets — bank accounts, investment accounts, retirement accounts, real property, vehicles, business interests, personal property of value — must be listed with current values. All debts — mortgages, car loans, credit cards, student loans, personal loans — must be listed with current balances. Failure to disclose an asset is perjury under Nevada law.
